HADCO has partnered with Umpqua Health Alliance and Oregon Health Authority to provide Health-Related Social Needs (HRSN) services to eligible OHP members living in Douglas County. HRSN is a non-medical benefit program available to OHP members who meet specific eligibility criteria. HRSN services help address barriers to health, like unstable housing. HADCO’s service coordination is focused specifically on housing-related needs, like support with rent and utilities, tenancy support, and home changes for health and safety.

Not all OHP members will qualify for HRSN housing benefits. Due to federal requirements, to qualify for HRSN housing benefits you must have specific health, living, and financial conditions. Our HRSN Community Services Coordinator can help OHP members determine if they meet the eligibility criteria and help them apply for services.

We are gathering referrals from HADCO and DMPM tenants over the next few weeks. We expect our HRSN coordinator to start contacting interested individuals on Monday, December 15th. Individuals will be contacted based on the date we received their questionnaire. We are also accepting referrals from Umpqua Health Alliance and Oregon Health Authority.

It’s important to note that HRSN housing benefits aren’t meant for emergencies, like pending eviction. If you have been given a court date because you’re behind on rent, please call UCAN at (800) 301-UCAN (8226), or visit their website, ucancap.org, instead of applying for HRSN services. If you need support with nutrition or climate support (air conditioners, heaters, air purifiers, etc.), please contact Oregon Health Authority or Umpqua Health Alliance for a referral to an HRSN provider specializing in those services.
